Referral information

Referrals to our service can be made via phone, email or through our webpage via the contact us link. Self referrals are accepted, however we may need to be in contact with your GP to obtain relevant medical information. Medical review is required to undertake an OT assessment and clearance must be obtained from VicRoads prior to completion of the on road component. 

To arrange for an assessment we require information about your medical history, licence details, driving history, and an understanding of your future driving goals. You can refer to our list of frequently asked questions which will help answer any questions you may have about the process.
